| Dear Participant You have been asked to take part in a questionnaire to talk about your views of your life and experiences. The aim of this questionnaire is to find out about what interests you, the types of things that may cause you to feel stressed. We would also like to know what you would like to have in your community to support you as a young person. Taking part in this questionnaire is voluntary and you can leave at any time. Anything that you say to us during your participation will remain anonymous and confidential: this means that what you say will remain between the team who are involved with this survey. The only time that this confidentiality will have to be broken is if you disclose exact details about a crime, or if you tell us something that makes us think that you or another person may be at risk, we will have to pass this information on. Anything else that you say to us will remain within the focus group and our team. The results of your questionnaire will be put together with the results from the other young people’s questionnaires from across Kent, and some focus groups that we have ran, and all of the results will be put into a big report. We may use quotes, but your name will not be mentioned, instead we will give you a different name (a pseudonym). If you agree to take part in this questionnaire, please select ‘Agree’ below. Thank you!
